Who We Help
We support children, adults, and older people who need tailored, compassionate nutrition care to manage everyday health and nutrition challenges. This includes people living with physical, intellectual, sensory, or psychosocial disability, those with chronic illness, complex medical needs, or mental health conditions, and individuals experiencing difficulties with eating, weight management, digestion, swallowing, or food tolerance.

Submit Referral
Use our referral form or send participant details and documentation via email. We’ll confirm receipt promptly.
Initial Review & Intake
Our team reviews the plan, funding and goals, then contacts the participant or nominee to clarify support needs and preferred appointment format (telehealth, in-home, clinic).
Service Agreement & Scheduling
Once details are confirmed, we prepare a service agreement and match the participant with the right dietitian based on needs, availability and location.
Consultation & Ongoing Support
The first appointment is arranged, followed by ongoing care tailored to goals. We’ll provide clear reports and documentation to help inform plan reviews and demonstrate progress.
Communication & Updates
We keep support coordinators updated with progress, clinical findings and recommendations, helping you advocate for your participant’s needs within their NDIS plan.
