Failure to attend policy
If you are going to be late for an appointment, please let your dietitian know. Failure to arrive at the appointment within 10-15minutes of your appointment, will result in forfeit of session & incur a cancellation fee.
Rescheduling policy
Any appointment that is rescheduled two or more times consecutively will be treated as a cancellation and charged a 50% cancellation fee. A new appointment will not be rescheduled until the fee is paid.
Cancellation policy
Cancellation or Reschedule with less than 2 business days notice will incur a 100% cancellation fee of service cost (plus travel cost if dietitian has arrived at property)

This form can be used for all referrals, including NDIS, Medicare, DVA, private payment, WorkSafe, TAC, and Aged Care Packages. It can be completed by the client, a health professional, family member, or support staff. Please have your funding details handy, and note that you can choose to complete a service agreement within this referral form.
